Once the name is changed what has to be done?
Once you have received your decree of name change, you will then have to obtain certified copies of the decree for passports, or social security and in some cases you can send photo copies of the name change decree to other agencies such as credit cards and banks. To change your birth certificate, you need to apply to the appropriate agency in the state/ country in which you were born and will probably need a certified copy of the name change decree along with a small fee. In no case can you change the father’s name on the birth certificate with a name change decree for a minor.
