Description

Notarization fee additional.

Your consultation with the Estate Planning Attorney will be scheduled within one business day from the date of purchase to ensure timely action.

There are two types of Powers of Attorney: one is for financial decisions and the other is for medical decisions.

For medical decisions, an Advanced Healthcare Directive or Medical Power of Attorney will designate a person or persons to handle your health care decisions in the event that you would be unable to make these decisions for yourself.

Frequently an accident, illness or age-related dementia can make it necessary for another person to make these decisions. You must be of ‘sound mind’ to request the preparation of an Advanced Healthcare Directive.

With an Advanced Healthcare Directive, you can decide the treatment you would receive in the event that you become ill or require life support. For example, if you choose not to be kept alive by life support, you can include this in the Healthcare Directive. You can also make decisions pertaining to other medical treatments and care.
